By now everyone knows about the little joke cameos such as the little Dark One appearing in the Caspian if Artyom takes drugs, but their appearances in the final level, The Dead City, are more than just a cameo. Basically they're there for the last half of the Novosibirsk level. Artyom isn't hallucinating cause of radiation sickness, it's the dark ones guiding him to his goal.
